A card is required to guarantee the booking. CHF 40 may be charged solely in case of no-show or cancellation with less than 24 hours notice. Please say whether delivering or collecting and identify documents/order; collections only once we confirm documents are ready. This is not a consultation or new order. - Required fields: `name` (Your name), `email` (Email address), `electronic_communications_consent` (I expressly accept electronic communications from ALPYREN concerning my appointment and document handover. I understand ordinary email is not end-to-end encrypted.), `late_cancellation_fee_consent` (I understand this appointment is free if attended or cancelled at least 24 hours in advance.
