● Bring a copy of your KY AOC494 and citation to this appointment, for KY DUIs.

● KY DUI assessment fee is $75 and is due at time of assessment.

● We accept the following method of payment:

Cash, CashApp, Venmo, or Money Order.

● This appointment is for an assessment, NOT classes. Class schedule will be discussed during assessment.
