● 24 HR NOTICE to cancel/reschedule is REQUIRED. Be considerate! Don't waste the appointment slot!!!

● Bring a copy of your KY AOC494 and Uniform Citation to this appointment, for KY DUIs.

● KY DUI assessment fee is $75 and is due at time of assessment.

● We accept the following method of payment:

Cash, CashApp, Venmo, or Money Order.

● This appointment request is for an assessment, NOT for class. Class schedules will be discussed during assessment.
