In order to obtain the most accurate figures and guidance during our conversation, you will need to have some information readily available:

Personal identifying information such as Social Security Number, Date of Birth & Medicare Beneficiary ID Number (if applicable)
First & last name of your primary care physician & all specialty doctors
Hospital preference
Current list of medications including full name, dosage & frequency taken
Your preferred pharmacy

A few things that may be helpful, but are not required:

Details about your current coverage (Deductibles, copays, out of pocket maximums & premiums)
Details about plans offered to you either through COBRA or retirement plans
A general idea of your monthly budget for healthcare costs
