- Required fields: `name` (Your name), `email` (Email address), `origine` (Comment avez-vous connu Synoptïa ?)
- Optional fields: `origine-precision` (Si vous voulez préciser : quel assistant, quelle recherche, ou qui vous a parlé de nous ?), `notes` (Additional notes), `guests` (Add guests)
