SELF-PAY: 40 Min Primary Care & Wellness Visit
Working alongside your Primary Care Provider (PCP), we are responsible for health maintenance, disease prevention, chronic disease management, and health promotion. We offer support to both you and your Primary Care team to monitor the progress of your care. Our team is available to meet with you to further assess your condition, develop a plan of action that coincides with your established care plan, implement steps to carry out the plan, coordinate with your Primary Care team, and evaluate the effectiveness of the plan with your Primary Care team. Our team also provides advice and guidance on when you should seek immediate medical attention. (*Please Note: YOU HAVE CHOSEN TO SELF-PAY FOR THIS SERVICE. This service is usually covered by insurance. If you would like to bill this service through your insurance, choose the Primary Care & Wellness Visit appointment option that says Insurance Only.
