- Required fields: `name` (Your name), `email` (Email address), `company-name` (What's your company name?), `title` (How much is this problem costing you every month?), `intent` (How soon are you wanting to fix this problem with AI?), `smsReminderNumber` (Phone number (Text notifications))
- Optional fields: `guests` (Add guests), `notes` (What is the biggest problem in your business right now?)
