Dual Consultation
Use this link if you have both pre-65 health insurance AND Medicare consultation needs. This includes those already enrolled in Medicare and future enrollees.
Per CMS guidelines, after you schedule your appointment, I will send you an e-document called a Scope of Appointment to complete via email. This gives me permission to talk to you about your Medicare options. It must be signed by you at least 48 hours prior to our meeting. I thank you for your understanding on this matter.
If this meeting will include a spouse or other Medicare beneficiary, please let me know so that I can allot a little extra time for the second individual.
**Please note that by booking an appointment with me you are giving me explicit permission to contact you via telephone or email regarding Medicare, health insurance, or prescription drug coverage. We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE (TTY users should call 1-877-486-2048) 24 hours a day/7 days a week to get information on all of your options.
